Important News Covid-19 Virus

Derby Road site will be operating as an isolation site  from Monday 23rd March. The site will be manned by a full team of staff working via telephone and  e mail. We are also hoping to start using video consultations as well. All patients who need to be seen will be seen at  the Pinewood site where infection control protocols will be followed to prevent the spread of infection including the use of personal protective equipment such as masks, aprons, gloves and goggles. In the possible event that staff are forced to self isolate and / or become unwell , we will re -deploy staff from the Derby Road site as and when required. We hope in this way to preserve a resilient workforce for as long as possible. Any patient attending the Pinewood site will be given instructions to wait in the car park until asked to come into the surgery. ALL patient requests for appointments will continue to be assessed and prioritized by telephone and by e mail.  It is clear to us that many routine procedures and treatments will be delayed as we attempt to meet the needs of our most vulnerable and ill patients. We apologize sincerely for this and ask for the understanding of our patients.

What Data Leaves the Surgery

There are three systems in place where your medical information may be shared by the practice. These have all been set up with patient care at the centre. is designed to collate nationwide patient data for research purposes, for example. The Summary Care Record ensures emergency doctors have essential information about your medical history. The Enhance Data Sharing Model allows your computerised records to be shared between care providers, allowing consistent healthcare for those with complex and long-term conditions. More information about each initiative can be found below.

Information about you and the care you receive will be shared, in a secure system, by healthcare staff to support your treatment and care.

It is important that we, the NHS, can use this information to plan and improve services for all patients. We would like to link information from all the different places where you receive care, such as your GP, Hospital and Community Service, to help us provide a full picture. This will allow us to compare the care you received in one area against the care you received in another, so we can see what has worked best.

Information such as your postcode and NHS number, but not your name, will be used to link your records in a secure system, so your identity is protected. Information which does not reveal your identity can then be used by others, such as researchers and those planning health services, to make sure we provide the best care possible for everyone.

You have a choice! If you are happy for your information to be used in this way you do not have to do anything. If you have any concerns or wish to prevent this from happening, please speak to practice staff or download the opt out form below, complete it and return it to your surgery.

Summary Care Record

A Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had. You are able to view your own Summary Care Record on SystmOnline.

Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.

In an emergency, or when it is urgent, it’s important that doctors caring for you know about you and any important medical conditions you may have, or medicines that you are taking.

Doctors may not be aware of vital medical information if you are unconscious, for example. This information includes the medicines you are taking, if you have any allergies and if you have ever had a bad reaction to something.

This could make a difference to how a doctor decides to care for you, for example which medicines they choose to prescribe for you.

This means they can provide you with safer care during an emergency. Summary Care Records are also useful if you need care when your GP practice is closed or if you are away from home in another part of England.

If you do not wish for other health care providers to have access to your Summary Care Record, it's easy to opt out.

You can do this by filling in this form and handing it to reception at your registered branch.

This is the only way you can opt out of having a Summary Care Record. You cannot opt out by email or on the web because your GP practice needs to know that it’s you opting out (and not someone else who might try to opt out for you).

If you choose to opt out of having a Summary Care Record, and then change your mind later, we can still make one for you. You just need to let your GP practice know.

Also, if you choose to opt out of having a Summary Care Record after it has been made, we will hide your record so that it can no longer be viewed by NHS healthcare staff. We will only make your record available again if those who want to see it, ask to do so in writing and if an investigation has found it necessary, for example if there is a complaint about your treatment.

You can ask to have your record deleted. However, this may not be possible if the record has already been used to give you care.

Whatever you decide and when, the NHS will always give you the best possible care.
